There’s tragic news from Sunday’s Phish concert which took place at Chase Center in San Francisco. CBS Bay Area affiliate KPIX 5 reported one attendee died after falling from an upper seating level during the show, while an additional two others were injured in a separate falling incident.

Chase Center officials issued a statement to The Mercury News:

Two guests attending last night’s concert were seriously injured. The San Francisco Medical Examiner’s Officer has informed us that, sadly, one of the individuals has passed away. We extend our heartfelt condolences to the guest’s loved ones. We are working with the local authorities to determine exactly what happened, and will defer questions about the incident to the San Francisco Police Department.

A San Francisco police spokesperson told KPIX 5 that local authorities initially responded to a call for assistance around 8:55 p.m. ET last night. “Medics arrived and immediately provided medical treatment, but despite the efforts of the emergency responders the victim succumbed to his injuries and was declared deceased,” the spokesman confirmed.

While an investigation into the death will be conducted by The Office of the Chief Medical Examiner, the spokesperson told KPIX 5 there was “no evidence of foul play.”

Authorities were contacted again at approximately 9:45 p.m. to help two patrons involved in a separate fall. In the second incident, “an adult male who fell, along with another patron who was struck by the falling man, were taken to the hospital for non-life threatening injuries.”

Phish’s Sunday show was their second at the venue following their Chase Center debut on Saturday. The band’s current tour continues on Tuesday night in Eugene, Oregon.
